Snigs

I've got a 94 Econoline with a ton of miles. My problem could very well be my transmission is just flat worn out, but...
I've received several "diagnoses" that conflict.
When driving at slower speeds (generally), at times the vehicle will jump out of gear. Let off on the accelerator and it goes back into gear.
Reverse works fine. It shifts smoothly and correctly except occasionally it shifts from 1st to 2nd gear a little roughly. (Not jar your teeth rough, just rougher than the rest of the time). It NEVER does this under a load- starting off, going up steep inclines, etc... It rarely does it cruising at 55 mph. There seems to be no pattern as to time of day, number of miles driven that trip, or external temperature.
I've been told it's an E4OD transmission. It has *cough* 224,000 miles on it. My finances are extremely lean and considering I know absolutely squat about transmissions, I don't want to end up paying for a new one if it could be a minor problem. One fella told me the vacuum hose might be sucking air. The next told me that model doesn't have a vacuum hose. One told me if the transmission was actually going fout on it, it would occur all the time, not just some of the time. Another told me it can be sporadic.
I just would like someone who's not looking to help me part with my money to give me some ideas. Pretty please even.
As an after-thought- the fluid is full and doesn't appear to be burned or dirty. It and the filter were changed about this time last year.
ETA: And now I feel like an idiot, as this seems to be a board for manual transmissions. Please forgive. Moderators kick it wherever it needs to go and feel free to give me a swift kick as well.
